GUARANTEE, LIMITED WARRANTY, AND LIFETIME WARRANTY
TERMS
Abbott Diabetes Care Inc. ("Abbott") offers consumers who buy
("you") a new FreeStyle® Blood Glucose Meter or Precision® Blood
Glucose Meter (both singularly referred to as "Meter") within the
United States the following purchase protections.
90-Day Guarantee: If you are not fully satisfied with your
Meter, call Customer Care toll free at 1-888-522-5226 within 90 days of
purchase for a full refund. Your refund will be limited to the net amount you
paid (that is, subtracting any rebates). You must have a copy of the dated
itemized purchase receipt and the original packaging to obtain this refund.
Blood Glucose Meter Limited Warranty: Abbott extends a
Limited Warranty to consumers who buy a new FreeStyle® or Precision® Meter.
Under this Limited Warranty, your new Meter is covered for a period of five
years (FreeStyle®) or four years (Precision Xtra) from the original date of
purchase, as long as it has not been modified, altered, or misused. Under this
Limited Warranty, Abbott will replace, free of charge, the Meter if it is
defective in material or workmanship.
NO OTHER WARRANTIES, EXPRESS OR IMPLIED, ARE MADE. ABBOTT WILL NOT BE LIABLE
FOR ANY INCIDENTAL OR CONSEQUENTIAL DAMAGES RESULTING DIRECTLY OR INDIRECTLY
FROM THE FAILURE OF THE PRODUCT TO PERFORM IN ACCORDANCE WITH
SPECIFICATIONS.
Some states do not allow the exclusion or limitation of other express or
implied warranties or incidental or consequential damages, so the above
limitations or exclusions may not apply to you.
FRIENDS FOR LIFE PROGRAM SPECIAL OFFERS.
LIFETIME WARRANTY TERMS . This Lifetime Warranty is a
benefit of enrollment in Abbott's Friends for Life Program. Choice of
replacement meter will be at the discretion of Abbott.Free Lifetime Meter Upgrades : Meter upgrade requires
enrollment in the Friends for Life Program and use of your current Meter for at
least three years. Choice of replacement meter will be at the discretion of
Abbott.
FOR WARRANTY SERVICE , contact Customer Care at
1-888-522-5226 for assistance and/or instructions for obtaining replacement
meter. Abbott may require, as a condition of obtaining warranty service, that
you return the Meter postage prepaid, with proof of purchase to an address
specified by Customer Care.
Your Rights Under State Law . This warranty gives you specific legal
rights, and you may also have other rights that vary from state to state.
